The Aging and Disability Resource Connection (ADRC) helps you access in-home care services, daily living assistance, and other long-term services and supports (or LTSS): without having to call multiple phone numbers.

Where getting what you need becomes easier

You do not need dozens more phone numbers to call. You need one central way to get connected to resources that span across providers, programs, and community partners.

The ADRC operates as a No Wrong Door system. This means it serves as a single, coordinated point of entry for people with disabilities, older adults, and their caregivers to access needed long-term services and supports.

The ADRC helps people with disabilities and older adults access long-term services and supports (or LTSS) in areas like:

Personal care service referrals (for help with toileting, bathing, dressing, and moving throughout the home)

Cash aid assistance (when available and applicable)

Personal attendant service referrals (for help with things like driving, cooking, and housekeeping)

Support managing medication, finances, or other necessary tasks

Senior center referrals when those services are needed

Necessary home modifications to improve safety

Help applying to ADA-compliant paratransit programs to simplify mobility

Fall prevention programs that can help lessen the risks of falling

Meal delivery assistance by connecting you with local meal programs like Meals on Wheels

The Aging and Disability Resource Connection (ADRC) is a collaboration between the local Area Agency on Aging (AAA) and independent living centers. It acts as a one-stop shop for information and resource access.

Instead of you being responsible for cross-communication across agencies, the ADRC helps do the communication for you.

In this way, you can get more streamlined access to available private, public, and nonprofit programs: through a single coordinated system.
